1. I have read and understood the AEFL Student Code of Conduct and agree to always comply with its provisions whilst attending AEFL Clinics.

2. I undertake that if I am charged or convicted of any criminal offence before the AEFL Clinic that I will notify AEFL before continuing or attending.

3. I declare that the information I have provided to AEFL for the purpose of attending a clinic or post session activities is correct to the best of my knowledge. I understand that if I am found to have deliberately withheld or provided false information
